2013 Free Eye Exams for Service Dogs

The American College of Veterinary Ophthalmologists (ACVO) will host its 6th public service event providing free eye exams for service animals in May 2013.
Drs. Nick Cassotis, Nancy Cottrill, Ruth Marrion, Clara Williams and Anja Welihozkiy, along with over 200 ACVO board certified veterinary ophthalmologists across America and Canada will donate their services to provide complimentary screening eye examinations to Service Animals. The Service Animal’s owner/agent will incur no cost for these services. It is anticipated that through these efforts service animal health can be improved and potential disease averted for thousands of animals.

Registration is open April 1st- 30th, 2013

Steps to take:
DATES AND APPOINTMENT TIMES:
- Register your working or service animal between April 1st – 30th, 2013 to participate in the event during the month of May.
- Appointments will need to be made directly with your veterinary ophthalmologist only after you have registered.
- A link to a full list of participating clinics will be provided in your confirmation email.
REGISTRATION PROCEDURES:
- After you complete this form, call the ophthalmologist that you select from the list provided in your confirmation email to schedule your appointment.
- A confirmation letter will be emailed to you with a registration number and program instructions.
- Please have the confirmation number available when you make your appointment over the phone and bring the confirmation form with you along with your animal’s certification paperwork, to the ophthalmologist’s office.
SPECIAL NOTE:
- Those who were screened at previous events are welcome to participate again in the 2013 event.
- If you are visually impaired and need assistance completing this form please call the ACVO office at 208.466.7624, M-F between the hours of 9-4pm MST, we will do our best to assist you. Please do not call if you are visually able to register, we have very limited staff available for telephone calls.
